The ones that I bought were made in Sweden but these seem to be similar. I just found this site and am truly impressed by their products. I didn't see a price anywhere but here is the link to all their products.

Mine came with three different filters that you could change yourself, the below model seems to have 4 if I am not mistaken. Could be that you decide when you buy them how much reduction you want. If that is the case I recommend 9 dB.
The other filters blocked too much for my taste. 

The one that looks most similar to the one I have are the:

dB Life™ Sweet Tones Musicians EarpiecesNo. 11001 - 11004

Reduces all frequencies equally by 9 dB, 15 dB, 20 dB or 25 dB with corresponding Flat Attenuation Filters. Designed for musicians who want to hear sound without distortion but with less volume.

I found another one with what appears to be a more affordable solution. You get a free impression kit to do it yourself and three filters at a third the price that my orchestra paid when I had it done. Also they had to pay for the nurse to take the impressions.

I also put cotton in my left ear when I try to play. While my hearing is not as good as it was when I was younger, I seem to have sensitive ears. I remember a couple weeks ago, I got called out to help support a rather large grass fire in my neck of the woods. I was advised to proceed emergency traffic (lights and siren). Since the brush fire truck I was driving has NO a/c, the next 25 miles were horrible. Now when I have to drive emergency traffic, the earplugs go in.

You're smart for using them. It will pay off. I know so many musicians with tinnitus. Many have trouble falling asleep because of it.

That's what it's called.... My good friend had that and still has a lot of ear problems and it pretty much broke up his world renowned band as he was the lead singer. I never did buy earplugs but will get a pair now for sure.
