I'm starting in fourth position and the second repeat moves to third position for the high notes. But when to change back into first position is doing my head in.
Any suggestions?

I am completely mesmerized when listening to this.  I have faith in you Terry to play this well.  Please keep us informed on how it's going and which is the best way to play it.  The music you are playing from, Who did the arrangement?

Hi Kevin, I downloaded a PDF from violinonlinelesson.
I now start in III'rd posn. Go straight back to first, go to IIIrd whenever I want to avoid an open D, the near end is another shift to hit the high notes. My E sounds Orrible! So when my new strings arrive will borrow a camera to do a recording.
Beenn nearly a years worth of lessons now, so it'll be good to do a progress report

I play almost all of it in 1st and 2nd position with the E string part taking me way up there.
